Expert insights published on personalising selection of hybrid closed-loop system

An expert paper discussing pump, continuous glucose monitoring and whole system features to guide the personalised selection of hybrid closed-loop systems currently available in Italy has been published.

With multiple hybrid closed-loop (HCL) systems now available, tailoring the choice to clinical profiles and patient preferences can help optimise system selection.

This study provides practical examples of sound alignment between device features, as well as patient needs and preferences.

The paper presents a patient-centred framework that integrates device features, clinical needs, lifestyle and personal preferences to support shared decision-making, ultimately enhancing outcomes and quality of life for people living with diabetes.

HCL systems have transformed diabetes management by integrating continuous glucose monitoring (CGM) with insulin pumps and algorithm driven insulin dosing.

In type 1 diabetes, randomised controlled trials and meta-analyses consistently demonstrate increased time in range and reduced HbA1c with HCL compared to other insulin treatments, without an increased risk of hypoglycaemia.

Benefits have also been reported in individuals with type 2 diabetes, in pregnant women and across other groups of people living with diabetes.

The expert paper concluded: “The availability of different HCL systems offers a unique opportunity for a personalised approach to treatment. However, this diversity requires a thorough understanding of each system’s characteristics, strengths and weaknesses.

“Knowledge of the technical and functional properties of these devices is crucial for matching them to the specific clinical profiles of people living with diabetes, as to their individual needs and preferences.

“Technical expertise, awareness of patients’ needs and the ability to tailor therapy accordingly are key elements of modern diabetes care and represent essential tools for optimising outcomes and improving quality of life.”
